Services for Seniors
Social prescribing connects people 55+ with non‑medical supports in their community to improve overall health and well‑being. When a health‑care provider or community member notices that someone’s challenges are related to social needs, like housing, food, income, employment, or loneliness, they can refer them to a community connector. The connector works with the person to understand their needs, create a plan, and link them to local programs and services. They also help the person access those supports so they feel guided, not alone.
